For all of us, having an occupation means belonging to a group, feeling useful, and building an identity. When we talk about people with diagnoses such as schizophrenia or bipolar disorder, work takes on an even greater significance: it becomes a powerful tool for recovery and social inclusion.

The Ladder of Participation: Are you in charge?

To understand how work helps mental health, we need to talk about participation. Imagine a ladder: on the lower rungs, the patient merely receives orders or treatments passively, which we call the traditional model. As we climb these rungs, the person starts to be heard, but often this is just for show — what experts call tokenism, where the patient's opinion is gathered, but they still have no decision-making power.

True change happens on the highest rungs. It is when we reach partnership and citizen control. Here, power is shared equally between the healthcare professional and the person experiencing the problem firsthand. In the job market, this means that the person is not simply “placed” in a position, but rather decides their own steps, facing challenges with the necessary support to remain steadfast and autonomous.

Different Ways to Work and Start Over

Brazil has made significant progress in creating alternatives to ensure no one is left out of the job market. One of the most interesting modalities is Individual Placement and Support (IPS). Unlike endless training courses that often discourage the patient, IPS focuses on “place and train”: the person gets the job they want in a regular company (such as supermarkets or movie theaters) and receives specialized support while already working. This model has shown that people remain employed for much longer.

Another inspiring path is the Solidarity Economy, where social cooperatives emerge. In them, there is no pressure of unbridled competition, but rather self-management and solidarity. Projects like the POA Generation, in Porto Alegre, and the carnival block Suburban Madness (photo), in Rio de Janeiro, are examples of how crafts and music workshops can break down the walls of institutions and occupy the streets, returning citizenship and pride to the participants.

In addition, we have peer support, an idea based on the motto “nothing about us, without us.” These are individuals who have experienced psychosis themselves and now, properly compensated, help others who are beginning their recovery journey. This changes everything: the service user goes from being just someone who receives care to also becoming a caregiver, gaining a new professional identity.

Challenges, Laws and the Future of Inclusion

Sure, the path is not free of obstacles. Stigma and prejudice are still strong barriers in the corporate environment. Therefore, Brazilian legislation has been fundamental. Since the Psychiatric Reform, the focus has shifted from hospitals to the community. Today, laws such as Brazilian Inclusion Law and new quotas in public exams ensure that people with psychosocial disabilities have their reserved and respected space, with necessary adaptations so that they can compete on an equal footing.

Studies show that working improves symptoms, boosts self-esteem, and reduces the need for hospitalization. However, for this to work, companies must be prepared and the worker must have a solid support network involving family and healthcare professionals.

Initiatives such as the Rede Entrepares participatory research, which unites Brazil and Mozambique, seek precisely to listen to these community voices in order to build a more humane and down-to-earth mental health care system. The ultimate goal is a social transformation where every person, regardless of their diagnosis, can write their own story through work and active participation in society.

Have you ever stopped to think that work is much more than just a way to pay the bills?
